This Business Associate Agreement ("BAA") is entered into between Jack Beecher LLC d/b/a Denta ("Business Associate") and the entity subscribing to the Denta platform ("Covered Entity").
This BAA supplements the Terms of Service and governs the treatment of Protected Health Information ("PHI") as defined by the Health Insurance Portability and Accountability Act of 1996, as amended ("HIPAA"), and its implementing regulations.
